I thought this switch only had two choices 1 or 2 and no middle. Is the plastic cover of this video card white or red in colour?

If it is white it is a genuine Mac edition card, if it is red it was at least originally a PC edition. A PC edition card can be converted to have Mac firmware installed - like the white Mac edition card. Typically position 1 would contain one copy of firmware and position 2 a different copy. People upgrading it to have Mac firmware would do one copy and this then means the switch can be used to switch between the Mac and PC firmware.

I would try position 1 first and if the Apple logo shows during boot-up it is using Mac firmware, if it stays black until the login screen appears then that position at least has no Mac firmware. Then try the other position if necessary. It is not compulsory to install Mac firmware on this card as I mentioned without Mac firmware if you wait till the login screen it will work, it is merely that without Mac firmware you cannot see anything until the Mac has finished booting and therefore cannot access the Recovery HD partition or use FileVault or use the Option Key to select an alternate boot drive. (You can still use System Preferences and Startup Disk.)
